Application Process

To Request an Application

There are four ways you can request an application.

1. Print out application to the right and mail it to:
   Volunteer Services
   16251 Sylvester Rd SW
   Burien, WA 98166
2. Call (206) 988-5767, and ask for an application to be mailed to you. Currently, we are only accepting adult applications (18+ years and older). You may leave a message, but speak slowly and clearly and spell your name. Include your name, address, zip code and phone number.
3. Submit a written request, with the information noted above.
4. Send an email to kcouret@highlinemedical.org. Include the following information:
   • Full name
   • Complete address, including zip code
   • Phone number, including area code
   • Email address
   • Location choice, i.e., Main Campus, Specialty Campus or Clinic
5. Hospice volunteer applications can be obtained from Michelle Fawcett-Long at mfawcett-long@highlinemedical.org.
