Application Process

To Request an Application

There are four ways you can request an application.

1. Print out application to the right and mail it to:
   Volunteer Services
   16251 Sylvester Rd SW
   Burien, WA 98166
2. Call (206) 988-5767, and ask for an application to be mailed to you. You may leave a message, but speak slowly and clearly and spell your name. Include your name, address, zip code and phone number and whether you would like an application for an adult (over 18) or a teen (14-17).
3. Submit a written request, with the information noted above.
4. Send an email to kcouret@highlinemedical.org. Include the following information:
   • Full name
   • Complete address, including zip code
   • Phone number, including area code
   • Email address
   • Location choice, i.e., Main Campus, Specialty Campus or Clinic
5. Hospice volunteer applications can be obtained from Michelle Fawcett-Long at mfawcett-long@highlinemedical.org.
