Patient Conferences

To ensure we stay connected, our inpatient and outpatient teams get together weekly to review care and planning for all inpatients and new or complex outpatients.

Sometimes things get complex for patients and families, as well as the healthcare team. Then our dedicated oncology social worker arranges a family conference where the patient, family and healthcare team meet to manage the plan of care. Keeping patients and families engaged in the process is vital.
