As an HMSO patient, you have the right to:

  • Receive considerate, respectful, collaborative care.
  • Receive prompt, quality medical care.
  • Receive clear, complete information concerning your health.
  • Know the names and responsibilities of all those caring for you.
  • Experience private and confidential handling of your medical records.
  • Review and correct your medical records at any time.
  • Seek another medical opinion or change healthcare providers.
  • Refuse treatment or leave the clinic even if this is against medical advice.
  • Receive a copy of your clinic bill and an explanation of all charges.

As an HMSO patient, you have the responsibility to:

  • Make sure that the medical history you provide us is as accurate and complete as possible.
  • Actively participate in making decisions about your healthcare.
  • Follow the instructions and advice of your healthcare providers, or tell them if you do not wish to do so.
  • Tell your healthcare providers if you notice any changes in your health.
  • Provide the clinic with complete and current information about your insurance, your address, and your telephone number.
  • Arrive for your appointment on time, or call 24 hours in advance if you must cancel or reschedule.
  • Treat our staff courteously.
  • Pay your bill in a timely fashion, or seek assistance with payment options.
