As an HMSO patient, you have the right to:
- Receive considerate, respectful, collaborative care.
- Receive prompt, quality medical care.
- Receive clear, complete information concerning your health.
- Know the names and responsibilities of all those caring for you.
- Experience private and confidential handling of your medical records.
- Review and correct your medical records at any time.
- Seek another medical opinion or change healthcare providers.
- Refuse treatment or leave the clinic even if this is against medical advice.
- Receive a copy of your clinic bill and an explanation of all charges.
As an HMSO patient, you have the responsibility to:
- Make sure that the medical history you provide us is as accurate and complete as possible.
- Actively participate in making decisions about your healthcare.
- Follow the instructions and advice of your healthcare providers, or tell them if you do not wish to do so.
- Tell your healthcare providers if you notice any changes in your health.
- Provide the clinic with complete and current information about your insurance, your address, and your telephone number.
- Arrive for your appointment on time, or call 24 hours in advance if you must cancel or reschedule.
- Treat our staff courteously.
- Pay your bill in a timely fashion, or seek assistance with payment options.
